Kindly grant me an emergency leave today. Reason being my wife is Pregnant and admitted to the hospital as for her complicated labor condition.

I have received a call from my family members that my wife has requested my presence at the delivery room.

My leave status will be well informed to the HR Dept. From the date of leave______ till the date _______ I return for Work. hope that you will grant me an emergency Leave on urgent basis.

Taking note of the present and the past six months situation you have been given three disciplinary notices with regard to your behavior. The management appreciates and respects the employees who are punctual and concern about attendance and deadlines. We will loose our clients if we cannot offer them dependable service. This is an unacceptable pattern of behavior from your side. Thus, we can no longer continue your employment at ___________ (Name of the company).

This decision is effective immediately. You will be paid two weeks salary.
