Sub: Maternity leave
Dear [Ms./Mr. Last name],
My fourth child is due to come into this world next Thursday, and the family has been having sleepless nights because of the excitement!
Although I was earlier asked to work right until the due date if I so preferred, I have now been advised to check myself into the hospital a couple of days beforehand. This may have something to do with a minor complication I experienced during my last pregnancy.
I am therefore writing you this letter to request a 5-day leave of absence which comes into effect from this Friday.
