Consent for Minor to Travel

Use this letter when you are the parent or guardian of a child who is younger than 18 and are providing consent for that child to travel without you. Include pertinent details, such as the child's full name, age, date of birth, where he or she will be traveling to and from, dates of travel, whether he or she will be traveling with anyone else, and if so, who the travel companion will be. Most likely the child will carry a copy of this letter with him or her on the trip, and you should also keep a copy of this letter at your home in a safe place. I hereby provide consent for my son, Scott Gregory Barringer, to travel with his 11th grade band class from Philadelphia, Pennsylvania to Washington, D.C. on July 25, 2010. He will be returning to Philadelphia on July 29, 2010. Scott is 17 years old, and his date of birth is June 3, 1993.

He will be traveling with a group from Willoughby High School and will be under the supervision of the school's band teachers and chaperones. In the event of an emergency, I can be reached at telephone (202) 555-1212.
